The five questionnaires below help me to help you. They give me a general overview of your current psychological landscape. Some people love a questionnaire, some people hate them! I ask everyone to please complete Questionnaire 1 before the initial consultation. The rest are optional should you wish to provide me with more information.

 

 

Naturally the questionnaires can take a bit of time to complete however the more information you can give me now, the less sessions you'll need to pay for later! Most therapists don't run tests on their clients like this, but if you went to a doctor with a medical problem, you'd be concerned if they didn't run a series of tests to check for problems and rule things out! Many people even report that doing the questionnaires helps them to start getting some insights. The questionnaires are private, mobile-friendly, and can be submitted entirely online. This questionnaire tests for implicit schemas currently held in your brain's limbic system. Implicit schemas are emotionally constructed unconscious belief systems that are projected on to reality and shape your experience of it. Implicit schemas can trigger thoughts, feelings, physical symptoms and sensations, and behaviours that make little sense to the logical, rational part of your brain, leading to a sense of confusion and disharmony within yourself.

This questionnaire tests for your attachment style. Attachment is a special type of schema that forms through relationships. Attachment schemas commonly get projected on to various relationships throughout life, from intimate relationships, to friendships and working relationships. While we think we see people as they are, what we frequently experience is what our brain is projecting on to people. This can have broad implications for how we, think, feel, and behave.

This questionnaire tests for your ontological perspectives. Ontological perspectives are essentially major structural belief systems regarding the nature of self, other, and world that invariably influence your cognitive style and shape your perception and experience of reality.

This questionnaire tests for possible genetic influences on your symptoms, behaviour, and perceptions of the world. This questionnaire tests for your personality type. Personality could be thought of as existing on a continuum ranging from flexibility to rigidity. The more rigid our personality the more difficulties we are likely encounter in life. Our personality profile is typically made of implicit schemas, attachment style, ontological beliefs, and genetic factors. This questionnaire helps me cross-reference the other questionnaires.

This questionnaire tests for the functional state of your nervous system. Under certain circumstances, traumatic events or overwhelming life-experiences can cause a dysregulation in our Autonomic Nervous System, preventing a return to equilibrium or homeostasis. When this happens we can experience a sense of disconnection from life, discomfort in our own bodies, or dysregulation in our day-to-day mood.
